In the past, most of the rehabilitation services are only focused on body functioning and the rehabilitation routines within hospitals, not much focus on the discharge planning of patients. – and now I realize after this course that I will consider whether the life participation abilities of the patients have improved under different environments and circumstances. You have to think outside the hospital. Have to think on team work and coordination, and standard documentations.
